History of Present Illness

History of Present Illness or HPI, is the meat and potatoes of most visits. Recording a patients HPI during a visit is a great way to connect with patients. HPI can lead to a better understanding of what the patient is going through. When recording the HPI, physicians want to know:

1. What's wrong?
2. What are the symptoms?
3. What has happened leading up to these symptoms?
4. What treatments have been tried so far?

Importance of HPI
